S.2376 - Federal Government - A bill to establish a demonstration project to provide for patient-centered medical homes to improve the effectiveness and efficiency in providing medical assistance under the Medicaid program and child health assistance under the State Children's Health Insurance Program.

Medical Homes Act of 2007 - Directs the Secretary of Health and Human Services to establish a three-year demonstration project under titles XIX (Medicaid) and XXI (State Children's Health Insurance Program) (SCHIP) of the Social Security Act for the implementation of a patient-centered medical home program to improve the effectiveness and efficiency in providing Medicaid and SCHIP assistance to an estimated 500,000 to 1 million targeted beneficiaries.
